Your听Primary Name听is display-only. If you need to update this, submit the听Change of Name form.

Your Chosen name will appear as your name on class rosters and may be used in other University systems/communications.听听University applications like听听and听WebReg听will use the chosen name from the Human Resources system if one exists.听 Students that are also staff may need to follow the instructions on听听to update their chosen name across systems.

To enter a Chosen Name听in 91原创SIS from the Student Homepage:

• Click the Personal Information tile.
• On the Personal Details screen, if you do not have a Preferred name listed:
  • Click the plus icon to add a name.
  • The听Edit Name听window will open.
    • Select听Preferred听for听Type.
    • Provide your Chosen Name information for the other required fields.
    • Click the听Save听button.
• On the Personal Details screen, if you already have a听Preferred听name listed:
  • Click the name with听Preferred听displayed in the听Type听column.
    1. The听Edit Name听window will open.
       1. Update your Chosen Name information.
       2. Click the听Save听button.

To add a听Diploma Name听(how your name should appear on your diploma) see important information at听Diploma Name and Address.

See听Update your Emergency Contact information. You must provide at least one personal phone number. It is vital to keep this information up-to-date.

• The听Home Address + button听allows you to enter a new Home address with an effective start date, or you may edit your current address by clicking on it.
• Use the听Add ... Address button听to add a new address type.
• To add a听Diploma Address, see听Diploma Name and Address.

• An emergency contact is someone the University will contact should something happen to YOU. Those contacts with a name ending with (MC) are your听"If Missing"听contacts.

• The Privacy Restrictions link allows you to set general restrictions regarding specific types of personal information.
• See听听to view or update restrictions, along with听setting Exceptions for Dean's List and/or Graduation publications.
